Reporting Metro 2 Data to Equifax Canada

In order to set the companies up for reporting in our systems, for each company being reported, the following documentation is required:
For reporting evaluation:- Master Reporting Agreement- signed by the company (with their legal business name) as the owner of the data being reported- General Data Reporting Application - to provide information on the data being reported (please note there is a separate application for rental payment data reporting)
For set up in our systems:- Application for Service - Reporting - Provides our credentialing team with information needed for setup- A copy of the business license or articles of incorporation - supporting documentation required by the credentialing team- A copy of the consumer consent language when obtaining consent to report to the bureau - supporting documentation required by the credentialing team
Third party Authorization:- Client Authorization Form - Signed by the client authorizing Switch Labs to report data on their behalf.
